Exchange Agreement/Code of Conduct

At your orientation we will give you an Exchange Agreement/Code of Conduct form and ask that you and a parent sign it. This is what's on the form:.

 

I,(your name) agree to participate in The Incredible Exchange program at (Exchange/Volunteer Site). I will give my time, energy, talents and efforts for a total of (25 to 50) hours. My mentor and I will schedule my hours each week. I agree to assist in volunteer activities listed on the Exchange Site Menu as well as other tasks that may be assigned by my mentor. At the completion of my contracted volunteer hours, I will receive (Exchange Credits=Hours) Exchange Credits to be used at (name of where you will use your Exchange Credit Certificate).

I also agree to the following:
1. To attend an orientation with the Exchange Site mentor.
2. To keep to my volunteer schedule as much as possible. I will communicate with my Exchange Site mentor as needed regarding my schedule.
3. To call The Incredible Exchange office to report any changes during the session.
4. To follow the Rules/Policies established by the Exchange Site, The Incredible Exchange, and Minnesota 4-H.
5. To arrange my transportation to and from the Exchange Site.
6. To complete a pre and post survey.
7. To conduct myself according to the Minnesota 4-H Code of Conduct. The full Code is available online here or through the office of The Incredible Exchange. The code operates in conjunction with the Minnesota 4-H Youth Development Program principles and the regulations of the specific programs and activities involved.

Minnesota 4-H Code of Conduct Summary:
Participants will:
o Accept responsibility for representing themselves, The Incredible Exchange, St. Louis County, Minnesota 4-H, and University of Minnesota Extension with dignity and pride.
o Be punctual for scheduled activities, participate fully in planned activities, and be courteous to others.
o Refrain from leaving the Exchange Site while volunteering there, unless written permission from the parent or legal guardian is given to the mentor prior to the leave.
o Abide by all laws of the State of Minnesota.
o Refrain from disruptive and/or inappropriate behavior and clothing.
o Recognize the rights of others and respect the property of others.
o Refrain from the use of and/or possession of any weapons.
Parents and legal guardians attending or participating in 4-H youth development programs are also requested to conduct themselves according to the Minnesota 4-H Code of Conduct and assist and support youth in their efforts to adhere to the Code.

My parent/legal guardian and I have read, discussed, accept, and will abide by the full Minnesota 4-H Code of Conduct as referenced above. We also understand that infraction(s) of the Code may cause termination from membership in The Incredible Exchange program and forfeiture of my exchange credits. We also understand that being sent home at my parent/guardian’s expense may also result. We agree to accept the appropriate and logical consequences of my actions.

To Parent/Guardian: “I have reviewed the above agreement with my child and agree to provide the necessary assistance to ensure my child completes the requirements of The Incredible Exchange.”

The signature of the participant and a parent/legal guardian are required.
